you need to ask them. sure you might tell them to speak up and let you know if it's too much or too little during a massage... but most are prone not too talk. just ask them throughtout te session - like "so, how's the pressure? is it too light or heavy".
Discuss with the client at the start of the session before massaging, and check during the massage if the client wants if softer or firmer.
An experienced practitioner should be able to do this although they may not get this right 100% of the time.
During my treatment I can usually gauge what technique is appropriate + pressure etc. based on pattern recognition, presenting complaint, case history & what I have seen before.

Usually I start softly and slowly, and then incease pressure, you can tell if they tense up..
You ask your client. Each and every person's perception of touch is different. So what most people may consider an acceptable level of pressure, others may not be able to tolerate. If the client has a pre-existing medical condition, they may find even the slightest touch very sore. Communicating with your client is crucial.
