Body wash question, help please?!

I went to dermatologist today because I got some bumps in my chest and back, and she gave me a wash and a cream. She said " the wash is going to be like your soap" and after your shower wait till chest and back are dry and apply cream.

My question is: when I'm taking the shower should I apply it on affected area and the let the water hit me. (I don't think this is right, cause the water is taking the wash out of my body )

or

after I close the shower, then apply it on affected area and then wait til it dries so I can put the cream.
