Answers:
Not everyone with ovarian cancer needs chemo.
However, for arguments sake lets say we know for sure some cancer was left behind and the patient refuses chemo and/or radiation. In this case the cancer will spread until the patient dies.
No cancer is predictable, they often surprise you and can spread anywhere, but some cancers tend to spread more orderly than others. Ovarian cancers tend to seed on the omentum and in the subdiaphragm area, even in the early stages. Distant mets and organ invasion is less likely than spread over the serosal surfaces. The deadly potential of ovarian cancer is often encasement of the intra-abdominal organs causing the patient to die from intestinal obstruction and inanition. It is a terrible way to die.
Health Question & Answer

It depends on the type of the cancer. There are some severe types which spread so fast, through the blood stream, but some kinds take more time to spread. I think that first of all, this person has to be sure the tumor is really malignant. For a while, chemo is the fast way to destroy it, but another techniques are being experimented with a relative success. Nowadays there is also a kind of vaccine being used, but I do not know for sure its precision. The most important is never give up, never to surrender at the first challenge.Health Question & Answer

You don't know, I mean it is never predictable.

It could go anywhere in the area, cervix next, then the birth canal, and even the rectum/anus, I wouldn't want that.

There is no need for this. Tumors can be excised if not having chemo, but you get chemo afterwards, or radiation which might cause sterility. Most probable outcome, is a hysterectomy in 1 - 3 years, and very serious, no-turnaround within 5. Lymph nodes pick it up as well. Health Question & Answer

Cancer continues to spread until it is stopped. If you refuse to have chemotherapy, unless a miracle happens, it will spread, and continue to spread. And yes, they can die. If left untreated, they WILL die. Talk to your oncologist, if you are still against chemotherapy, to maybe find another treatment.Health Question & Answer
