Answers:
there have been extremely rare cases of sepsis from a unit of blood that was contaminated with bacteria. I've never seen a case in my life, but have read of it happening many years ago. It's a good bet this did NOT happen to your daughter in the year 2008.
Blood is never cleaned before transfusing. It is heavily screened for all relevant pathogens when it is collected, in order to ensure a safe blood supply.
presently, the estimated risk that you will receive ANY infection from a transfused unit of blood is 2.8 per hundred thousand units of blood transfused -- that is indeed a very safe blood supplyHealth Question & Answer

My condolences on the loss of your daughter.
It is unlikely that the blood transfusion had anything to do with it because all blood is screened very carefully. Health Question & Answer
