Part of my molar tooth near the back of my mouth fell out - what should I do?!

Question: Part of my molar tooth near the back of my mouth fell out - what should I do.?
A fourth of my back molar fell out, but it doesn't hurt at all.

I also had a really bad toothache there like a month ago, but since it went away, I neglected to go to the dentist - what should I do though.? I probably don't have time to go to the dentist till Wednesday, and I was wondering what I could do in the meantime.?Health Question & Answer

Answers:
From the sound of it, you have either lost a filling or fractured one of the cusp of the tooth and the piece has finally broke off. The fracture was the reason for the tooth ache a month ago and since it's broken free now, the pain you experienced upon chewing has stopped. This is typical of a fracture or loose restoration; when you chew or bite down on the tooth, the pieces or filling will separate from the tooth, causing pain, once it breaks off it doesn't push upon the solid piece of tooth any more allowing stimuli and liquids to enter inside the tooth.

Now the problem is, you have a rough broken tooth (probably cutting or irritating your tongue) that isn't protected by the hard enamel covering that was once there, leaving the tooth with open, with exposed dentinal tubules that are normally covered and protected. These dentinal tubules work like transmitters, they carry any and all stimuli (hot/cold) directly to the nerve of the tooth and can cause hypersensitivity and pain which can traumatize the tooth further causing pulpitis to occur. This pulpitis is an inflammation of the nerve and can actually kill the tooth.

Since you are seeing your dentist early this week, you should be fine if you can keep your tongue from rubbing the rough spot, avoid hot, cold or sweet on the tooth and keep it very clean especially after eating or drinking anything. It will either need a restoration or a crown, this will depend on just how much tooth structure has been lost with the fracture.

If the tooth starts to bother you or hurt you will need to see your dentist sooner, (they will work you in for this) this is an indication the nerve is reacting to the break and it may require a root canal. You may want to take some Ibuprofen such as Advil or Motrin 400-600 mg every 6 hours to help keep the tooth settled down. I think your tooth will be fine since you aren't experiencing any serious pain at the moment, just be sure to see your dentist to have it restored. Leaving it as it is will cause the tooth to eventually abscess which will require a root canal and a crown. Better to take care of it now while it's still small. Health Question & Answer
