Answers:
You know or your dentist knows that it's time for wisdom teeth to be extracted when they are causing you pain from being impacted/have no room to grow in, when they are causing your teeth to shift, or when your gums start to get infected as they grow in.

Your dentist should always do an x-ray when considering extracting your wisdom teeth. X-rays are necessary to determine exactly what's going on underneath your gums and to find out the angle that the teeth are growing - in order to remove them correctly. Sometimes dentists extract two at a time (on the same side of the mouth - upper and lower), if the teeth are ready to come out. However, if the procedure takes a long time or its too painful for the patient, the dentist will do separate extractions for all.Health Question & Answer

From xray only you will come to know that how far they are and whether they have sufficient space and their angulation is right for comming out. If the space is not sufficient then they can be impacted and if above things are not good then you may have to get them pulled out.
