Depends on a lot of things..
If you get a through and through shot to the face, that doesn't kill you, it will hurt like @@#@##$.. If you graze the skull, same applies..
If you use a large enough caliber directly placed to the head, then the bullet travels faster than your nerve endings can relay the message to your---well what was your brain.. That's why it is said to be painless..

The brain itself has no feeling, so that wouldnt cause pain.. If you take a bullet to your head and live, Im sure there is some pain.. If you die, that is why it is painless because death happens quickly..Health Question & Answer
