Yes, they both die, unless doctors can separate the two bodies quickly enough. Otherwise, the living twin's heart would have to pump blood through both bodies, and no heart is strong enough for that. Depending on what the twin died of, its dead body could begin decaying, which would quickly kill the other twin, anyway, just as a person who is not a siamese twin will die after one of their body parts dies and turns gangrenous.
