A doctor determines if you are at risk to yourself if you state that you are planning on harming yourself and you HAVE A PLAN to do so and tell them.
My doctor knows I self-harm and I talk about suicide alot. But they don't put me in the hospital because I don't have a specific plan. Once when I was serious and had a plan, they baker acted me, for the 72 hour forced hold.Health Question & Answer
