Paranoia can be caused by anxiety or panic disorders, by severe trauma, any kind of abuse, and can also be caused by actually being stalked/hunted/chased/having someone after you. Really intense, reccurent nightmares can also cause paranoia.
Some medical conditions can have paranoia, to varying degrees, as a symptom.
Some medications, prescription and "over-the-counter" kinds, as well as just about all illegal drugs can cause paranoia that lingers even after the person stops taking/abusing whatever substance.

Nobody seems to know, but there is a well accepted theory that their is a connection between schitzophrenia (means paranoia - nothing to do with "multiple personalities" which may not even exist).
That it has to do with a common cat virus that reacts poorly in the human brain.
