You will fill out a form before you see him, so don't worry.
You should find the whole session really helpful, and might be surprised at how easy it was.
Try not to worry about the meeting, just go and be honest.
I think you should ad that your mother is not supportive. The dr needs to know things like this very much.

Your therapist will usually let you talk about whatever you want and then ask you questions as you talk. They will not probe unless you give them reason to. I went to counseling when I was 16 and severely depressed. I think it's good to have a counselor as a teenager. You need someone to talk to who WONT judge you. That's right, they do not judge. also, they keep what you say confidential. If you reveal you are currently being abused physically/sexually, they have to by law notify authorities for your safety.
