There are many theories as to what causes it but there are no definitive answers yet. What the doctors do know is that it can be hereditary meaning if one person in your direct family line ie mom or dad has it or one of their family members has it or had it then it increases your chance to be effected by it. Bipolar Disorder is characterized by a chemical imbalance in the brain with the serotonin receptors and the amount of serotonin that's being put out by the axon of the nerves. At times the axons are putting out too much at other times not enough. By being on antidepressant medication along with medications to effect serotonin production and absorption many Bipolar Disorder patients are able to live relatively normal lives. It is the patients that don't take their medications properly that have problems, usually the younger less experienced patients. They start feeling normal because of their medications and think that they are cured and stop taking them which then sets a cycle in motion and in a few short hours or days or weeks depending on the speed of their cycle they are back into their manic and/or depressed states. Often it goes unnoticed until someone in the patients family receives a bill for a shopping spree or the patient tries to hurt themselves and has to be taken to the hospital. That is the most I can give you other than to add that often the medication cocktails these patients are on often changes because the body can and often does develop a tolerance to the medications.
