What do physicians check when they are filling out a physical form (for sports)?!


Question: What do physicians check when they are filling out a physical form (for sports).?
Is there any one who has been to a physician for a physical.?

I've haven't been to a physician for awhile and I don't know much about physical forms either. I just want to know what they check/do.(please give me good details)

sapphire, There are some differences among health forms physicians use. Your school nurse may be able to provide you with a blank form. Usually checked are your past health history, possibly questions about family diseases. The physical will likely include examination of height, weight, posture, head, eyes, ears, nose, throat, neck, spinal column curvature, lungs, heart, possibly abdomen, hernias, feet. NO pelvic exam is included. When a male physician does the exam of a female, another female attendant is normally present or may be requested, if you so desire.Health Question & Answer
