You just took it out too soon. Tampons are very absorbent and can hold a lot of blood. But if you take them out while they are still dry it will feel like you are pulling all of your insides out with it. I would recommend you only use a tampon on the days you bleed the most. Otherwise use a pad.
You can get toxic shock syndrome if you leave a tampon in too long. It is recommended that you never leave one in for more then six hours. For that reason I wouldn't wear one at night when you go to bed.
Every thing will be fine, it is scary the first few times.

yes, this is normal. how long did you leave it in.? usually, when this happens, it means there isn't enough moisture in the tampon, therefore feeling as if it was tearing coming out. i usually leave mine in for about 5 hours if i'm really light. also, playtex is the best brand by far. most comfortable and works best. maybe try a smaller absorbancy or leaving it in longer. good luck.Health Question & Answer
You probably just didn't have it in long enough. This happens to a lot of people.
As a general rule, you probably shouldn't use tampons after like the third day; there's just not enough flow. And wait for at least four hours before changing them. Trust me, you'll KNOW when it should and shouldn't be changed.

if you don't leave it in long enough, it won't get soaked enough with blood. then, its dry and won't come out.
sit on the toilet and completely relax your whole body while you take it out. pull at a 45 degree angle going forward.Health Question & Answer

Another reason along with what everyone else said is that you need to relax the muscles down there. If you are too tense then it will hurt to take out the tampon.
