If you like them, then just pick an appropriate time and ask them. They are usually pretty good about these types of things since they are instructors preparing you for that field.
I would like to request a letter of recommendation from you for a position that I am seeking with _______? Would you be willing to provide me with this letter?
first and most important: ask WELL in advance. give them plenty of time to write the letter. that way they can think of the best things to say about you without being too rushed.

basically, if the person is familiar with you, i would just walk up to them when they dont look too busy and politely ask if they are willing to write one for you. just be polite.
Type this out and have the professor sign it.

dont wait. some instructors have limits. the sooner the better. just go up to them and ask casually; no big deal
It's always good to let the instructor know why you have chosen him/her for the recommendation. You should talk about what you learned in the class and why it's relevant to whatever you need the recommendation for. If it's a large class or if the instructor doesn't know you well, you should also talk about your performance in the class. Also, remember to thank him/her for the letter afterward. I've always asked for recommendations over email because it allowed me to be more formal and organize my thoughts, but asking in person works, too.
Just ask your instruction; that's it.
Just ask him or her directly or go to his office to ask him for a letter.
