How do I apply for my brother to be institutionalized in a mental home?!

Is he really mentally ill? If so, contact his doctor and ask for help. No help? If he is a danger to himself or others he can be involuntarily temporarily committed for observation. Ask your county health department how to do this.
I would think the process begins with the psychiatrist. He would have to fill out the required paperwork regarding your brother's condition. He then would have to coordinate the paperwork with the institution's admitting process.

Look online for good ones but make sure you do background checks on these before you apply your brother to them because we dont live in a safe world. Ask as many question as your prossbile can make sure his therepist can visit and you can to as much a often what type or illness do you brother have.

Some states have group homes for people with mental illness. They are a little less restrictive than hospitals, but the group home will pretty much take over all aspects of his life (money, medication, daily activities).
