In some states the dental hygienist is licensed to give anesthetic for pain. Topical ointments are also available. Just voice this concern with your hygienist and they will make it as comfortable for you as possible. If the hygienist is not licensed to do this, have them ask the doctor to anesthetize you. It can make your visit much more comfortable.

I am answering this based on my own experience. If you have sensitive teeth and gums a deep cleaning may hurt. But, if it hurts the dentist can give you a shot. The longer you wait the worse it is.
