I cant speak for everyone else but my Ex-wife is dead, so she may be burning in hell, maybe that is why she would be an old flame?

It's just a real old expression. He lit up your life once, and now the flame has been extinguished.
i would say ex is usually used for things or people who're not with us or arent a part of our lives anymore..so it makes sense to call a previous flame an ex cuz with that it makes the person someone who u knew and loved and yet that person no longer is a active part of your life and theres room for someone eles to step in and take his or her place..:) just a guess
