on a drug bottle it's telling you how the expressed concentration was calculated.

This is a ratio of "volume to volume". If the concentration of an ingredient is 2% w/v, there are 2 grams of that ingredient in a total of 100mls. If it is 2% v/v, there are 2 mls of that ingredient in 100mls.
