Here's what I did. I typed all of my child's medical information on paper, and had it notarized for free, in my bank. I gave this form to my friends that took my daughter on a hiking trip for 2 weeks. She did have an asthma attack, and they used this form for hospital treatment. They called me and informed me that she was ok. Make sure in the form that you specify that you are giving Power of Attorney to whomever, and you need to type the dates for how long they will have Power of Attorney.
