As long as your husband goes with the child, they do not need a permission slip from you.

I worked in a large children's teaching hospital and I remember what it was like when a child was admitted for treatment and the legal guardian or parent was not available. Some physicians and surgeons are really strict about operating or treating without such a consent form, signed, and in place.

Now, if I am ever taking care of my small grandchildren and their parents are out of the country, I always make sure that my son picks up this consent form from the hospital to have just in case. Having it on hand always seems to ensure that it will never be needed.

So, to get such a form, just go to the emergency department of the hosptial, tell them that you are leaving the country and you wish to have a "Consent For Treatment" form so you can have it filled out and signed in case one of your children needs attention. If you are not able to do this, just check out the site below, print off the consent form and have it signed and authorized and have ready to take when ever the children need treatment.
