Answers:
Change doctors! Seriously. You need to find someone to treat the illnesses and not just the pain. I am prescribed one type of anti-depressant for the Chronic Fatigue Syndrome and another for the Fibromyalgia.

My CFS and Fibro do not go hand-in-hand; they are free-standing diagnoses. I, like you, have several other illnesses that i deal with, including osteoarthritis, general anxiety disorder, panic attack, all symptoms of lupus, but no positive test results, prosthetic jaw on the left side of my face due to a defective TM joint implant, and on and on.

Exercise, as much as it hurts, is great for the Fibro. I've found therapeutic massage to be beneficial for both the CFS and the Fibro.

But, first and foremost, find a new doctor! If you're near west Texas, i can recommend a wonderful one out there.

If you are not being helped by Vicodin which is actually Hydrocodone there are other options too Like oxycodone also known as percocet but its addictive . Ask your doctor about Neurontin it is actually a class of antidepressant but it works really good with narcotic analgesic pain medications and helps good control over neuropathic pain.
There are some other choices if you really want to go ahead with major surgical pain interventions they start with epidiral steroid injections and end with spinal cord stimulator or a morphine pump. They are specialised devices to cut down your oral intake of these meds which are going to damage your liver sooner or later but again pain control is must. you can ask for lidoderm patches also.
above all is your own will power . I know at some point the will power does not work but you still have that and for that reason you asked this question on this platform. I respect your courage for that. This is not the end of the world options are still available . Look for a good pain physician . by the way ultram is weak opioid analgesic and when you are taking vicodin its waste to take ultram. I dont want to point on the physician but its not useful at all .You will need clonidine for fibromyalgia , muscle relaxant like zanaflex or robaxin and anti inflammatory like celebrex if you have gastric irritation, or simple Diclofenac sodium known as voltaren.
Please feel free to e mail if you need more info .I can help you if your case is workers comp .
Stay healthy thats what I can say at this moment.
All the best!

Pain management

Patients with fibromyalgia generally see many physicians before receiving a correct diagnosis. Patients may seek medical advice for more than 5 years before a correct diagnosis is made, and more than 50% of patients receive a misdiagnosis and may undergo unnecessary surgery.
The most widely acknowledged biochemical abnormality associated with fibromyalgia is abnormally low serotonin levels.
Several medications should be avoided or used carefully. To date, the U.S. Food and Drug Administration has not approved any drug for the specific treatment of fibromyalgia.
Medications useful for treating fibromyalgia include the following:

Sleep problems

Antidepressants (eg, trazodone, SSRIs, dual-reuptake inhibitors [SNRIs], tricyclic antidepressants)

Anticonvulsants (eg, clonazepam, gabapentin, tiagabine)

Nonbenzodiazepine hypnotics (eg, zolpidem, zaleplon, eszopiclone)

Muscle relaxants (eg, cyclobenzaprine, tizanidine)

Dopamine agonists (eg, pramipexole)

Depression - Antidepressants

Pain

Nonsteroidal anti-inflammatory drugs

Analgesics (eg, tramadol)

Other

A wonderful book is available to help you find and fix the CAUSE of your problems. It is written by Sherry Rogers, M.D. She is a specialist in environmental medicine, a clinical researcher and teacher, and has over 35 years of clinical experience with complex cases like yours.

The book is available at Amazon.com or from the publisher at 1-800-846-6687.

I highly recommend this book.

Best wishes and good luck.

You need a pain receptor blocker. That's what worked for me. You should find a new doctor, this one is just drugging you to shut you up not treating your problems.
