When you deliver a baby (in the field anyhow) you clamp the umbilical cord on two ends, next to the baby and close to the placenta. then you cut between the two clamps. all of the afterbirth, placenta, cord, and whatever other discharge has to be kept so you can bring it to the hospital for biopsies.
Some people want to keep their baby's placenta, besides those rare occasions after the biopsies and whatever else, the placenta is eventually incinerated.
