Alternately, it could be advanced celiac disease (i.e. autoimmune reaction to wheat; seek treatment (call the doctors to find the appropriate diagnosis venue first if you won't pick an E.R.) and then change your diet for that one. It's odd for celiac to comprimise the upper gastric valve (which most often closes in emesis) but it happens. However, there's no reason it can't simply be a lung occlusion revealed by the convulsion you speak of, loose glass in the chest, being shot, complication of acid reflux disease, or for a common malady associated with dissatisfactory meals:

A stomach ulcer.

That clears up like being shot most times, but if you live and can prepare a good examination (e.g. by not eating anything but lettuce and spinach salads with ranch dressings, and dilute OJ with calcium) then you may be able to avoid the problem in the future.

If the person has a drinking history, it could very well be hemmorrage from gastic varices. This is also a precursor to liver failure requiring transplant if left untreated.

If you can rule out eating disorders and alcohol/drug abuse then it could be any number of things. Some possibilites include a bleeding ulcer or an irritable bowl disorder such as Crohn's disease. You need to go see a doctor for a proper diagnosis and for treatment.
