It depends on the cause. Many sinus infections are viral, in which case the only cure is a tincture of time. Bacterial infections should respond to antibiotic therapy and fungal infections to antifungal therapy. Symptomatic treatment includes pain relievers and possibly decongestants like Sudafed (provided you don't have a contraindication like high blood pressure). Finally, some people have anatomical conditions that predispose them to recurrent problems and may require surgical correction.

Not only can bacteria colonize the nasal sinus, but several fungi can as well. If the full course of antibiotics doesn't cure it, I'd ask for a nasal lavage, with a gram stain. This will differentiate the organisms. A culture may not be accurate due to the antibiotics.
