Consult a medical doctor, that specializes in gastric medicines.
Your BMI must be over 40, with no co-morbidities (high blood pressure, sleep apnea, diabetes) or 35-40 with one or more co-morbidities. You will probably have to prove to your insurance company that you have failed at diet and exercise. My insurance company (VISTA) is currently making me do a 6 month physician supervised diet before they will approve.

There is lots of great info on that website. I would first start this journey by talking to your primary care physician (PCP). If he/she is not supportive of you and you really feel that you need this surgery, get a new PCP. You will need all the support you can get.
