Obviously, the pain medications that he is on at the moment is not doing any good since he is still in so much pain. He is on some good pain medications already. The Fentanyl patch gives off certain amount of medicine through the skin and usually last for 3 days. The dilaudid he takes are pill form, right.? Those don't get absorb very quickly but nevertheless effective in a few minutes. He may have to get some pain medications through an IV (intravenous catheter). Now to do that, he may have to go to the hospital. Try to explain to him that the only way he can get relief from the pain is to go to the hospital where everything is accessible. The medical team can determine what can be helpful for him to minimize the pain. Part of the problem too is that, he maybe also experiencing depression from him being sick specially with pancreatic cancer. He's probably manifesting that depression through anger towards the doctors and the nurses or even the family members. Just be supportive. is it maybe a muscular type of pain.? could you give him a back rub (or hire a massage therapist.? or put a heating pad on it.? or else give him some nyquil so he can sleep through it.? (but don;t do that without consulting a doctor/nurse because if he's taken something already we might not want to mix two different medications) or do something with him to keep his mind off of the pain. put in his favorite movie.? A real simple solution :if he suffers from pancreas cancer he needs very strong painkillers. Just call 911:have him taken to the ER
