The white thing under the foreskin is called smegma. It is quite common. Some boys have it and others not. If washed daily, with soap and warm water it can stop altogether.
The build up of smegma is a good place for bacteria to breed. It is the smegma that is causing a mild infection on the glans penis and this is why you are having these red spots.
As for the itchy testicles, it is as a result of excessive sweating. Try to trim the hairs and make sure you wear cotton underwear so that the sweat can be absorbed. Applying talcum to the scrotum would also reduce the excessive sweating.
