Pull it back just enough that it doesn't interfere with aiming, so the stream doesn't actually touch the foreskin. There's no harm in peeing through the opening, as long as you wash thoroughly in the shower.

I just pull it a little bit, without exposing the glans completely so that urine doesn't get caught under the foreskin.
