You need a professional cleaning because anything that you can do at home will not be enough. After the cleaning, if there is not too much bone loss it is possible for the tooth/teeth to tighten up, but it is not a certainty.Health Question & Answer
You can't tighten them back up because the periodontal disease has actually destroyed the underlying bone that anchors the teeth.
Getting the disease taken care of by having a deep cleaning may stop the problem from getting any worse but the bone can't be regrown.
Sometimes the dentist can anchor the affected tooth/teeth to other stronger teeth in some cases.
